- Candidates may reside anywhere on the East Coast (EST time zone).
- Must hold an active, unrestricted New York RN license.
Key Responsibilities
- Conduct telephonic assessments using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS).
- Perform in-person assessments for non-MLTSS members to evaluate medical needs and determine appropriate service referrals, including:
- Adult Medical Day Care
- Pediatric Medical Day Care
- Personal Care Assistant (PCA) Services
- Nursing Facility Custodial Care
- Personal Preference Program (PPP)
- MLTSS Enrollment
- Work with diverse populations, including pediatric and medically complex members.
- Coordinate care with members, authorized representatives, primary care providers, and interdisciplinary care teams.
- Schedule and participate in interdisciplinary care meetings while advocating for members to ensure safe transitions and appropriate services.
- Develop individualized care plans and authorize services within MLTSS/FIDE program benefits in a cost-effective manner.
- Accurately document assessments, care plans, and member interactions in the electronic health record.
- Utilize strong critical thinking and problem-solving skills in daily case management activities.
- Mentor new staff after achieving role proficiency.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of New York.
- Minimum 3 years of clinical RN experience in one or more of the following:
- Hospital
- Home Health
- Ambulatory Care
- Fluent in English and Spanish (reading, writing, and speaking).
- Associate Degree in Nursing (minimum).
Preferred Qualifications

- Crisis Intervention experience
- Certified Case Manager (CCM) certification preferred
